Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that Licensee's Mandated Reporter Child Care Providers training (AB1207) expired on 07/05/2025 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete the Mandated Reporter Child Care Providers Training (AB1207). Licensee agrees to send proof of their AB1207 certificate to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 08/07/2025.

Based on observation, the licensee did not comply with the section cited above as LPA observed several poisons/toxins in an area that was not key locked which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee immediatley moved poisons/toxins during visit. POC cleared during visit.

Based on record review, the licensee did not comply with the section cited above as LPA observed that C3, C4, C6, and C9 were missing proof of updated immunizations which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have C3, C4, C6, and C9's authorized representatives submit proof of updated immunizations. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing by the end of the business day on the POC due date of 04/25/2024.

Based on record review, the licensee did not comply with the section cited above as LPA observed that C7 was missing proof of the completed Consent For Emergency Medical Treatment - Child Care Centers Or Family Child Care Homes (LIC627) which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have C7's authorized representative complete the LIC627 and agrees to maintain proof in file. Licensee agrees to submit proof of the Plan of Correction (POC) by the end of the business day on the POC due date of 04/25/2024.

Based on observation and interview, the licensee did not comply with the section cited above as LPA observed and was told that children were being served 2% milk which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to stop providing 2% milk to children and agrees to write a plan of action detailing Licensee's plan to ensure compliance with the above regulation. Licensee agrees to submit proof of the Plan of Correction (POC) by the end of the business day on the POC due date of 04/25/2024.

Based on observation, interview, and record review, the licensee did not comply with the section cited above as LPA observed that the assistant present, S1, was not associated to the facility which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have S1 associated to the facility and agrees to ensure all adults, living or working at the facility, are both fingerprint cleared and associated to the facility. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing by the end of the business day on the POC due date of 04/12/2024.
