Based on interview and record review, the licensee did not comply with the section cited above as LPAs observed that S3 was missing proof of Tuberculosis (TB) clearance that meets regulation requirements which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain proof of S3's Tuberculosis (TB) clearance that meets regulation requirements. Licensee agrees to send proof of S3's TB clearance to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 10/28/2025.

Based on interview and record review, the licensee did not comply with the section cited above as LPAs observed that Licensee's Mandated Reporter Child Care Providers Training (AB1207) expired 08/2025 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete the Mandated Reporter Child Care Providers Training (AB1207). Licensee agrees to send proof of AB1207 certificate to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 10/28/2025.

Based on interview and record review, the licensee did not comply with the section cited above as LPAs observed that S3 was missing proof of the measles immunization which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain proof of S3's measles immunization. Licensee agrees to send proof of S3's immunizations to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 10/28/2025.

Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that C1 was missing from the roster and that children Licensee indicated were no longer enrolled were missing the "Date Left" section which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to complete/update and maintain roster with currently enrolled children. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 10/28/2025.

Based on interview and record review, the licensee did not comply with the section cited above as LPAs observed that C4 was missing proof of the 15 minute sleep checks which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to document 15 minute sleep checks for C4 and all infants enrolled in care. Licensee agrees to send proof of the 15 minute sleep checks for C4 to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 10/28/2025.

Based on observation, the licensee did not comply with the section cited above as LPA observed a damaged slide in the on-limits backyard which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit a written plan of action on how compliance will be maintained with the cited regulation by 10/14/2025. Licensee agrees to submit proof of repair or removal of the damaged played equipment for the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 10/28/2025.

Based on observation, the licensee did not comply with the section cited above as LPA observed several hazards in the on-limits bedroom, kitchen, and backyard such as a broken chair with jagged plastic, batteries, lighters, nails, and "keep out of reach of children" items which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee moved items during the visit. Licensee agrees to submit a written plan of action on how compliance will be maintained with the cited regulation. Licensee agrees to submit the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 10/28/2025.

Based on observation, the licensee did not comply with the section cited above as LPA noted a tide detergent was present in one of the on-limit rooms while the children were outside playing which posed a potential health, safety or personal rights risk to persons in care.

Resolution: LPA observed S2 remove the detergent from the on-limit area during visit. POC cleared during visit.

Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that C1 and C2 were missing proof of updated immunizations which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain proof of C1 abd C2's required immunizations. Licensee agrees to send proof of C1 and C2's immunizations to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 07/24/2024.

Provder and Employees should complete Mandated Reporter Training every two years following the date he/she completed their initial training. Based on record review, Licensee and Staff Member #2 Mandated Reporter Training expired. There is no documented on Mandated Reporting for Staff Member #3

Resolution: Licensee agrees to complete and submit proof of Mandated Reporter Training to the department for all staff by 07/31/2023.

Based on record review, the licensee did not comply with the section cited above in that Immunization (MMR and Influzena) missing for Staff Member #2, which poses a potential health & safety risk to persons in care.

Resolution: Licensee agrees to obtain and submit proof of Staff Member #2 Immunization (MMR and Influenza) to the department by 08/14/2023.
