Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that C2 was missing proof of the Consent For Emergency Medical Treatment (LIC627) which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to have C2’s authorized representative complete the LIC627 and maintain proof at the facility. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 01/09/2026.
Based on observation, the licensee did not comply with the section cited above as LPAs observed several poisons/toxins as well as several items with "keep out of reach of children's" labels on top of the refrigerator in the on-limits kitchen area that can be reached by the tallest child present during the inspection which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee moved poisons/toxins as well as items with "keep out of reach of children's" labels to locked off-limit area during visit. Licensee agrees to submit a written plan of action on how compliance will be maintained with above listed regulation. Licensee agrees to submit proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 12/21/2024.
Based on interview and record review, the licensee did not comply with the section cited above as S1 did not have proof of CPR/1st Aid and was left alone with the children briefly while Licensee stepped out to get food for the children during the inspection which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to have S1 enroll in an EMSA approved CPR/1st Aid course and agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 01/10/2025.
Based on interview and record review, the licensee did not comply with the section cited above as LPAs observed that S1 did not have proof of the Measles immunization or Pertussis immunization on file which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to obtain proof of S1's Measles and Pertussis immunizations. Licensee agrees to send proof of S1's Measles and Pertussis immunizations to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 01/10/2025.
Based on record review, the licensee did not comply with the section cited above as LPAs observed that C4 was missing proof of the Consent For Emergency Medical Treatment - Child Care Centers Or Family Child Care Homes (LIC627) which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to have C4’s authorized representative complete the LIC627 and maintain proof at the facility. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 01/10/2025.
Based on interview and record review, the licensee did not comply with the section cited above as C1 was missing proof of 15 minute sleep checks and Licensee admitted that they have not been documenting the 15 minute checks which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to document 15 minute sleep checks for C1 and submit proof of the 15 minute checks to Community Care Licensing by the end of the business day on the Plan of Correction (POC) due date on 10/10/2023.
