Based on observation, the licensee did not comply with the section cited above as LPA observed drawer where knives were kept, in the on-limits kitchen area, was unlatched and the area which contains poisons/toxins to be unlocked which posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee moved knives to an inaccessible area and locked the poisons/toxins during the visit. Licensee agrees to submit a written plan of action on how compliance will be maintained with the cited regulation. Licensee agrees to submit proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 05/13/2025.
Based on interview and record review, the licensee did not comply with the section cited above as LPA observed S1 did not have the correct Mandated Reporter training on file, S1 completed the General training rather than the Mandated Reporter Child Care Providers Training (AB1207), which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to have S1 complete the Mandated Reporter Child Care Providers Training (AB1207). Licensee agrees to send proof of S1's AB1207 certificate to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 05/13/2025.
Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that roster was not updated as children who no longer attend, per Licensee, do not have the "Date Left" section completed which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to update and maintain roster with currently enrolled children. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 05/13/2025.
Based on observation, the licensee did not comply with the section cited above, at the time of visit, poisons were located in the kitchen cabinet behind a child lock but children in care were outside in the play area, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agreed to immediately remove the poisons from the kitchen cabinet and placed them in the first floor hallway closet that is key locked during the visit.
Based on interview and record review, the licensee did not comply with the section cited above as S3 is missing proof of TB, MMR, and Influenza vaccines which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensees agree to obtain proof of the TB, MMR, and influenza vaccine/influenza declination statement for S3. Licensees agree to send proof of the immunizations to Community Care Licensing by the Plan of Correction (POC) due date of 01/25/2024.
Based on record review, the licensee did not comply with the section cited above, S1 and S2 Mandated Reporter Training was completed 08/01/2021 and was not renewed, S3 Mandated Reporter Training is missing, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to have S1, S2, and S3 complete Mandated Reporter Training and provide proof of completion to the LPA via email by COB 01/25/2024. www.mandatedreporterca.com
Based on interview and record review, the licensee did not comply with the section cited above as both co-licensees are missing current non-expired EMSA approved CPR/1st Aid cards which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensees agree to enroll in a EMSA approved CPR/1st Aid course for both co-licensees. Co-Licensees agree to send proof of enrollment to Community Care Licensing by the Plan of Correction (POC) due date of 01/25/2024. www.emsa.ca.gov
