Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that C1 did not have proof of the LIC9227 on file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have C1’s authorized representative complete the LI9227 and maintain proof at the facility. Licensee agrees to send proof of the Plan of Correction (POC) to Community Care Licensing (CCL) by the end of the business day on the POC due date of 01/09/2026.

Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that C1, C2, C3, and C4 were missing proof of updated immunizations which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain proof of C1, C2, C3, and C4's required immunizations. Licensee agrees to send proof of C1, C2, C3, C4's updated immunizations to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 01/09/2025.

Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that C4 was missing proof of the Consent For Emergency Medical Treatment - Child Care Centers Or Family Child Care Homes (LIC627) which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have C4’s authorized representative complete the LIC627 and maintain proof at the facility. Licensee agrees to send C4's completed LIC627 to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 01/09/2025.

Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that S1 was missing proof of Tuberculosis clearance which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain proof of S1's Tuberculosis clearance and maintain proof at the facility. Licensee agrees to send proof of S1's Tuberculosis clearance to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 01/09/2025.

Based on observation and interview, the licensee did not comply with the section cited above as LPA observed that C1 was asleep in a crib that had loose fitting sheets on the mattress which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to read the Infant Safe Sleep regulations and agrees to submit a statement of understanding regarding Infant Safe Sleep regulation 102425(a)(3) to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 01/09/2025.

Based on interview and record review, the licensee did not comply with the section cited above as the last firedrill, conducted 01/20/2023, was not conducted within the required six month time frame which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to conduct a firedrill and submit proof of the firedrill to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 10/04/2023.

Based on interview and record review, the licensee did not comply with the section cited above as S2 was missing proof of the Tuberculosis vaccine and Pertussis vaccine while S3 was missing proof of the MMR vaccine which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain proof of the required immunizations for both S2 and S3. Licensee agrees to submit proof to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 10/04/2023.

Based on interview and record review, the licensee did not comply with the section cited above as both C1 and C3 were missing proof of the AFFIDAVIT REGARDING LIABILITY INSURANCE FOR FAMILY CHILD CARE HOME (LIC282) which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have C1 and C3's authorized representatives sign the LIC282 and agrees to send proof to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 10/04/2023.

Based on interview and record review, the licensee did not comply with the section cited above as both C1 and C3 were missing proof of the PARENT NOTIFICATION ADDITIONAL CHILDREN IN CARE (LIC9150) which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have C1 and C3's authorized representatives sign the LIC9150 and agrees to send proof to Community Care Licensing (CCL) by the end of the business day on the Plan of Correction (POC) due date of 10/04/2023.
