An unannounced, on-site Monitor Inspection was initiated and completed on January 21, 2026. The on-site inspection began at 11:50 am and ended at 2:40 pm. The inspector reviewed compliance in the areas listed above. There were 32 children present and 8 staff. The inspector reviewed 4 children?s records and 4 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI, and this single inspection report reflects findings related to licensing standards and subsidy standards. Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Licensing Inspector Anderson conducted an unannounced Monitoring inspection in response to a self report of an incident that occurred at the center on April 30, 2024. Licensing Inspector initiated the investigation on May 1, 2024 and conducted the onsite inspection on May 15, 2024. Findings were reviewed with the center on July 15, 2024. The Licensing Inspector reviewed 1 child record and 1 staff record, along with any requested program records submitted by the facility to determine if required documentation was complete. A monitoring inspection was initiated on April 9, 2021 and concluded on April 12, 2021. A self-reported incident was received by the department regarding allegations in the areas of hazardous substances and supervision. The director was contacted by telephone to conduct the investigation. Staff were interviewed and video of the incident was reviewed. The evidence gathered during the investigation supported the self-report of non-compliance with the Standards, and violations were issued.
