The on-site inspection began at 3:55 p.m. and ended at 4:50 p.m. The inspector reviewed compliance in the areas listed above. There were 25 children present and six staff. The inspector reviewed five children?s records. Staff records will be reviewed at the main office on a later date. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business December 3, 2024. A POC submitted after this date will not appear on the public website.
A mandated monitoring inspection was conducted on December 12, 2023 from 3:40 P.M.-4:40 P.M. on site and concluded on 12/13/2023 at administrative offices for staff record review. There were 12 children present, ranging in ages from five to nine years of age, with three staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition.
