A subsidy health and safety inspection (SHSI) completed on 10/22/2025. The inspection started at 11:20 am and concluded at 11:50 am. There were (2) children present and (1) caregiver; the point total was (5). The inspector reviewed (3) children?s records and (3) caregiver/household members records on-site. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the program.

An unannounced annual Subsidy Health and Safety Inspection ( SHSI) was completed on 12/9/2022 from 11:10 am-12:05 pm. There was 1 child in care directly supervised by the vendor. The child was playing with a soccer ball, cars and helping with the inspection. The home, emergency preparedness planning, supplies and drills were reviewed. A sample of child information, provider trainings and household member information was also reviewed. Areas of non-compliance identified on the Violation Notice.
