An unannounced, on-site monitoring inspection was conducted on 7/21/25 from 11:25am to 12:20pm. The inspector reviewed compliance in the areas listed above. There were 38 children present and 9 staff. The inspector reviewed 3 children?s records and 3 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

An unannounced inspection was initiated and completed on 6/23/25 with a local agency, in response to a complaint received by the licensing office on 6/17/25 relating to allegations of supervision, nutrition and special care and provisions. The inspector reviewed 3 children?s records and 3 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program. Though not a part of the original complaint, a violation that was not reported was found related to records, and is documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 7/1/25. A POC submitted after this date will not appear on the public website.

A subsidy health and safety (SHSI) inspection was conducted on 7/22/24. There were 38 children present with 12 staff. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. An announced initial inspection was conducted today from 10:30 am to 12:40 pm. The physical plant, staff qualifications, background checks, policies and procedures and the emergency preparedness plan were reviewed. Classrooms were measured to determine the center's capacity. The classrooms are equipped with supplies, equipment, and age appropriate activities for children. 8 infant cribs were inspected, and crib compliance documentation was reviewed. The outdoor playground equipment and surface was inspected. Many areas related to the supervision and care of children could not be inspected due to no children in care at this time.
