An unannounced, on-site monitoring inspection was initiated and completed on October 3, 2025, as a part of the licensure period. The on-site inspection began at 9:50am and ended at 10:35am. The inspector reviewed compliance in the areas listed above. There were two children present and two caregivers; the point total was six. The inspector reviewed two children?s records and four caregiver/household member(s) records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The inspector reviewed compliance in the areas listed above. There were five children present and two caregivers; the point total was 16. The inspector reviewed five children?s records and four caregiver/household member(s) records on-site. This inspection included document review, tour of the facility, interviews, and observations. A monitoring inspection was initiated and concluded on November 18, 2021. There were three children(10pts) present, ranging in ages from 7 months to 18 months, with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and four staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. An announced initial monitoring inspection was conducted from 1:00pm-2:15pm. A tour of the home was conducted, records were reviewed, and the provider was interviewed. There were four children present (12pts), ranging in ages from 5 months old to 2 years old, with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. Four household/caregiver records were reviewed in addition to the family pet's record.
