The center shall obtain documentation of additional immunizations once every six months for children under the age of two years. The most recent documented immunizations for child #4 (23 months) is from when the child was 12 months old.
The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented. Child #2 has a diagnosed food allergy, per the childs physical exam. Child #2 was not included on the centers allergy list.
Childrens records shall contain all required information. Child #6s record does not contain documentation of an annual review of the childs emergency contact information. The most recent documented review of the childs record is dated 2/5/2024.
The center shall obtain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Child #2 has a diagnosed food allergy, per the childs physical exam. The center has not obtained a written care plan.
Cleaning and sanitizing materials shall not be located above or next to food, food equipment, utensils or single-service articles. A spray bottle of a bleach cleaner was observed stored on a shelf with disposable drinking cups in the Preschool 1 classroom.
