An unannounced, on-site monitoring inspection was initiated on February 20, 2025 and completed on February 28, 2025. The on-site inspection began at 11:30 am and ended at 12:00 noon. The inspector reviewed compliance in the areas listed above. There were 13 children present and 2 staff. The inspector reviewed 2 children?s records and 2 staff records on-site on February 20.2025. This inspection included document review, tour of the facility, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on March 7, 2025. A POC submitted after this date will not appear on the public website.
An unannounced inspection was initiated on November 8, 2024, and completed on December 13, 2024, with a local agency in response to a complaint received by the licensing office on October 25, 2024 relating to allegations of part of standard. The inspector reviewed 3 children?s records and 3 staff records on-site on November 8, 2024. This inspection included document review, tour of the facility, and interviews. The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to part of standard and are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. An unannounced monitoring visit was conducted during the after hours portion of the day. Four children stay for extended hours. Observed the transition between nap, hand washing, pickup and afternoon snack (bought from home). Reviewed 2 children's records, staff training, emergency preparation, physical space, medications -none were reported to be on-site and nutrition.Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Observed the transition between nap, hand washing, pickup and afternoon snack (bought from home). Observed children playing in the community center gym, transition back to the classroom, hand washing and lunch/snack. Food is bought from home. No medication was reported to be on-site today. Records are kept for staff for all Parks and Rec preschool programs at a central location. Staff have current Medication Administration Training certification, first aid and CPR, Daily Health Observation and annual hours.
