An unannounced, on-site monitoring inspection was initiated and completed on 10/3/2025, as a part of the licensure period. The on-site inspection began at 12:30pm and ended at 1:00pm. The inspector reviewed compliance in the areas listed above. There were 5 children present and 2 caregivers; the point total was 31. The inspector reviewed 2 children?s records and 2 caregiver/household members records on-site. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

An announced initial monitoring inspection was conducted 4/15/24 from approximately 9:35am-10:35am. There were no children present or enrolled. The home, 3 caregiver records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site. There are two rooms identified by use of children. There was an assortment of age appropriate materials for children. Areas of non-compliance are identified in the Violation Notice.
