An unannounced monitoring inspection was initiated on 01/13/2025 and completed on 01/21/2025 in response to a self-report received by the licensing office on 01/13/2025 relating to staffing and supervision. The inspector reviewed 1 child?s record and 1 staff record on-site. This inspection included document review, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. An unannounced, on-site monitoring inspection was initiated on 08/02/2024 and completed on 08/02/2024, as a part of the licensure period. The on-site inspection began at 10:15 am and ended at 12:15 pm. The inspector reviewed compliance in the areas listed above. There were 12 children present and 4 staff. The inspector reviewed 5 children?s records and 9 staff records on site. This inspection included document review, a tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. There were 10 children in attendance and a total of 4 staff present. 3 staff records and 6 children?s records were viewed. All areas of the center were observed including classrooms, bathrooms, and the playground. The children were observed taking a nap while the licensing inspector was on-site. Violations were cited as a result of this inspection. There were 8 children in attendance and a total of 4 staff present. 4 staff records and 4 children?s records were viewed. All areas of the center were observed including classrooms, the playground and bathrooms. The children were observed taking a nap, while the licensing inspector was on-site. Violations were cited as a result of this inspection. This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program. A monitoring inspection was initiated on 03/08/2022 and concluded on 031/08/2022. The director was contacted by telephone and a virtual inspection was conducted. There were 17 children present, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of 2 children?s records and 2 staff records were reviewed. The information gathered during the inspection determined a violation with applicable standards or law.
