During investigation, it was determined that caregivers did not provide adequate supervision, allowing a child to be left behind, alone in an empty classroom while the group of children left to another area/activity.

A child's finger was injured in the toddler restroom when a child placed their finger in-between the restroom door side stall and another child closed the door accidentally on the child's finger requiring medical attention. In conclusion of the investigation it was determined that two caregivers took 15 children inside the restroom at one time. Providers took the children to the restroom when it was advised by center policy that only half of the children to be diaper changed at one time.
