Based on information gathered during the investigation it was found that a person was present with children in care prior to the operation receiving a notification letter of eligibility.

The caregiver turned away from a child that required supervision during meals. This resulted in a lack of supervision as the child was able to reach for a cup of milk not assigned to them. The child was able to place the cup of milk against their lips and it is alleged child consume the milk due to the child?s severe allergic reaction symptoms.
