On the day of the incident, the child was seen swinging from the cubbies in the room after nap. The caregiver removed him from this activity. Later he was noticed to be pinching and scratching his right forearm and seemed to be in pain. The caregiver did not see the child fall but obviously, he hurt his arm somehow. She states the children always swing on the cubbies and she is always removing them and it is possible he went to swing again and fell and that is why she mentioned this to the parent and on the incident report. She states sometimes the children do fall when they do this but it is usually on their bottoms where they are not harmed. It is hard but the caregivers must supervise in a way so that children are not harmed.
Resolution: Corrected: 2021-05-07
Even after the operation became aware that the child was seen by a doctor for the injury of the arm, it was not reported to Licensing as required. The operattion states this is because they did not know the injury required medical attention until the time they were told the child had x-rays and did not show abnormal results; however, they were told they were taking the child for further evaluation. The operation now understands that they must report any medical attention sought after any possible injury.
