But after the parent picked up the child in the afternoon, parent found the bruises and took a picture. The operation failed to file an incident report and did not share to the parent. During the inspection, the operation was asked of any other incident report aside from the 3 dates provided which the parent also provided the same copies. This standard was found to be deficient.
