Concerns were verbalized to the director regarding the caregiver's aggressive tone towards the children in care. There was also another incident where the caregiver aggressively took a jump rope from a child in care. Collateral caregivers also reported their concerns to the director regarding discipline that was beyond the child's level of understanding and comprehension. These were not reported to CCR to investigate.

It was determined throughout the course of the HHS investigation that a caregiver utilized prohibited punishment with children in care. The caregiver pinched a child in care inflicting harm. The caregiver has also utilized aggressive and loud tones towards the children in care in an attempt to redirect the children, resulting in a chaotic environment. The caregiver has also utilized redirection not appropriate to the child's level of understanding or comprehension. The caregiver has also been observed inflicting pain on another child in care by taking away their jump rope while out on the playground.
