It was found during the investigation that at the time of the gas leak the person left in charge of the facility did not follow/know all the steps outlined in their Emergency Preparedness Plan. It was found that the planned evacuation routes for each class were not followed and instead of evacuating to the planned evacuation site stated in their Emergency Preparedness Plan they evacuated to the playground. It was also found that the gates on the playground were locked, and the employees present at the time of the evacuation did not know the location of the keys to unlock the gates therefore they were unable to exit through the playground gates.
