A review of the Employees Who Left Employment in 2025 revealed the center had not reported the number by January 15, 2025, as required.

There continues to be a broken door knob on the door that leads to the playground. Should the door be closed accidently, anyone outside can not open the door to go back inside the building as their is a push bar on the inside to get out. (This has not been corrected. ) The broken safe that held the door open was removed from the playground. There were no longer any dead bugs on the floor.

There is no handwashing sink in the infant room. The closest sink is in a restroom east of the classroom. The caregiver must walk out of the room leaving a child unsupervised to wash an infants hands. (Pictures on file.)

There are several riding toys that had pinch or shear points. This included broken or cracked tires, cracks along plastic parts of a tricycle. There is also a purple type worm/catepillar toy that has cracks in holes that children put their hands in. There is a scooter that no longer has a rubber cover and has a sharp edge that will cut a child's hand in not held properly. (This is a recite) as there continues to be broken equipment. There continues to be a broken door knob on the door that leads to the playground. To hold the door open there is a type of broken safe that is cracked and accessible to the children. There was a dead roach on the floor were children were passing through to reach their classrooms. (Pictures on file)

There was an employee in the building with no background check. The employee was working in the kitchen and had access to all the children in care.
