During a DFPS Investigation, it was determined that the two caregivers did not provide the required supervision necessary to ensure the safety of a child in care. A child was able to exit the operation, leave the property and was unaccounted for over an hour. The child was located by Law Enforcement more than two miles from the operation. The caregivers did not have physical proximity and auditory or visual awareness of this child.
