1.A TV that was mounted on the wall had an extension cord which was accessible to the children in care. The cord could be pulled, making the equipment fall on a child. The cords must be made inaccessible to the children in care. 2.The fence, around the outdoor playground, was observed to have an area that was not attached to the ground. This caused a hole that a child could gain access. The fence must be repaired to make this area inaccessible.
