During the inspection and review of staff files, individuals used in ratio did not have documentation of the required orientation training prescribed by the department completed within thirty days of the start date.

During the inspection, the preschool special education program did not meet requirements of section 3323.022 of the Revised Code and rule 3301-51-11 of the Administrative Code.Classroom 132am was identified as an integrated classroom however, 6 children with a disability and 5 typical children are enrolled. Classroom 130am was identified as an integrated classroom however, 7 children with a disability and 6 typical children are enrolled. Classroom 134am was identified as an integrated classroom however, 5 children with a disability and 4 typical children are enrolled.

During the inspection, it was determined the following requirement was not met: " Medication was not in original container or the container from which it was dispensed.
