In review of 25% of the children's records, it was determined that information had not been secured from the parent/guardian on the JFS 01234 Child Enrollment and Health Information For Child Care, as required, for the items in number 14 below. 1. During the inspection, it was determined that children were not protected from items or conditions which may threaten their health, safety, or well-being as noted in numbers 6 & 22 below: 1. Surge protectors/outlets did not have childproof receptacle covers.2. Open pull cords that are not closed loop. 3. Toys or other items small enough to be swallowed were present in the space where infants and/or toddlers were in care.4. Electrical/extension cords attached to an object that would not likely result in a severe injury if pulled. 5. Stacked chairs.6. Employee purses in the preschool classrooms. 7. Diaper bags.8. During the inspection, it was determined that unsanitary conditions, as noted in number 6 below, were in the Pre-K restroom:1. There was no liquid soap.2. There was no toilet paper.3. There were no paper towels.4. The toilet cleaning brush was accessible to the children.5. The plunger was accessible to the children.6. The toilet was not flushed.7. During the inspection, it was determined that a Serious Incident was not reported in the Ohio Child Licensing and Quality System (OCLQS), as required, by the program administrator or designee for an incident as listed in number 2 below:1. An incident, injury or illness that required professional medical consultation or treatment.2. An unusual or unexpected incident which jeopardizes the safety of a child, child care staff member or employee of the program.3. An incident defined as a serious risk non-compliance in appendix A to rule 5101:2-12-03 of the Administrative Code.4. The program did not submit the report in OCLQS by the next business day as required by rule. Submit the programs corrective action plan, which includes a statement that the program administrator or designee has completed the Serious Incident Report in OCLQS, to the Department to verify compliance with the requirements of this rule.

During the inspection, it was determined that a child was left unattended while inside the program as noted in number 1 below: 1.A child was left unattended once when a teacher took the class outside and the child remained inside the classroom. 2.A child was left unattended more than once. 3. Child(ren) left the group and were unattended. 4.Child care staff were using a baby monitor to supervise children. 5.Child care staff were using a walkie talkie to supervise children. 6.Child care staff were using mirrors to view children in another room. 7.Child care staff were using a video camera instead of physically being present in the room. 8.Other [ ]. Children must be supervised and within sight and hearing of a child care staff member at all times. Provide staff training. Submit the programs corrective action plan, which includes a statement that training was provided, to the Department to verify compliance with the requirements of this rule.
