During the inspection, it was observed that trampolines were used in the classroom that were not part of a child's IEP.

During the inspection, it was observed that electrical outlets on a surge protector were not covered when not in use and there was no documentation on file to indicate the outlets are tamper resistant.

During the inspection, the preschool special education program did not meet requirements of section 3323.022 of the Revised Code and rule 3301-51-11 of the Administrative Code. Rooms 11 and 15c are identifying as integrated classrooms, however they are meeting the PSE classroom requirements.
