During the inspection, it was observed that the surface of the outdoor play space did not offer protection from falls. There were 3 smaller moveable slides positioned on grass.

Additionally, the program was not complying with the Ohio Department of Health safe sleep procedures in that stuffed animals were observed in cribs and a mattress wedge was observed without written instruction.
