During the inspection and review of staff files, individuals used in ratio did not have documentation of the required orientation training prescribed by the department completed within thirty days of the start date.

It was observed or reported that the program used a prohibited disciplinary technique. Child was restrained by being buckled in seating and a stroller for longer than a brief period of time. The program reported this was in use at the beginning of the school year for the child's safety while seated, and it is no longer used.

During the inspection and review of staff files, the assistant who is the substitute for Lead Teacher for Room 35 did not have documentation to meet requirements for the substitute position.

During the inspection and review of documentation, the Lead Teacher for Room 31 has a caseload of 7 students with disabilities in a session. She is also implementing IEP's in Room 35. This places her caseload as more than eight children with disabilities in one class session.

During the inspection, the preschool special education program did not meet requirements of section 3323.022 of the Revised Code and rule 3301-51-11 of the Administrative Code. The assistant teacher has been assigned the role of lead teacher as a substitute in PS Room 35 . She has a high school diploma and has an current Educational Aide permit only issued by the State of Ohio.
