During the inspection, it was determined the program did not maintain a comfortable indoor environment as noted in number 1 below:1. The indoor temperature had fallen below 65 degrees Fahrenheit. 2. The indoor temperature had exceeded 85 degrees Fahrenheit, and fan ventilation or air conditioning was not provided. Submit the programs corrective action plan to the Department to verify compliance with the requirements of this rule.

During the inspection, individuals were not physically present with children to maintain ratio at all times. A ratio of 1 staff member to 19 children was observed when the preschool group was mixed with the kindergarten classroom.

During the inspection and review of staff files, individuals used in ratio did not have documentation of the required orientation training prescribed by the department completed within thirty days of the start date.

A child was reported or observed left alone or unsupervised. A kindergarten child walked a group of 5 preschoolers from the downstairs to the kindergarten classroom without a teacher present.
