During the inspection, it was determined the programs written disaster plan did not meet the requirement for training child care staff members and employees on the plan annually as noted in number(s) 2 below:1. Child care staff members and employees were not trained annually.2. Written documentation of the training was not kept on file. Submit the programs corrective action plan to the Department to verify compliance with the requirements of this rule.

During the inspection, it was determined that the program did not have designated storage areas for children's personal belongings. The rule requires that an area be designated where children can individually store their personal belongings. This area shall not block walkways or evacuation routes. Technical assistance was provided at the time of the inspection, and as discussed, please correct this rule noncompliance. During the inspection, it was determined that unsanitary conditions, as noted in number 6 below, were in the school age restroom:1. There was no liquid soap.2. There was no toilet paper.3. There were no paper towels.4. The toilet cleaning brush was accessible to the children.5. The plunger was accessible to the children.6. The toilet(s) were not flushed.7. During the inspection, it was determined that there was no written permission from the parents for school-age children to leave the program for specific activities and to return to the program. Written permission needs to contain the information noted below: 1. The childs name;2. The location of the activity;3. Arrangements for going to and from the activity;4. Start and end time of the activity;5. Time period for when permission is given.;6.
