During the inspection, it was determined that the provider was not meeting the following requirements as noted in number(s) [4 ] below : 1. The provider no longer resides at the licensed location.2. The licensed provider has additional activities/employment during operating hours, in that [ ]. 3. In review of the staff records, it was determined that child care staff member(s) or substitute child care staff member(s) did not meet the requirements for completing the online orientation training as noted in number(s) [ 3 ] below:1. Completion of the training was not verified in the OPR.4.
