591-1-1-.33(5)(a) requires that every calendar year after the first year of employment, all supervisory and caregiver Personnel, except independent contractors, Students-in-Training and volunteers, shall attend ten (10) clock hours of diverse training which is offered by an accredited college, university or vocational program or other Department- approved source. It was determined based on review of staff records that Staff # 3 with a hire date of July 6, 2013, and Staff # 7 with a hire date of February 26, 2019, did not have ten hours of diverse training for 2024 year.

591-1-1-.25(13) requires that potentially hazardous equipment, materials and supplies be stored in a locked area inaccessible to children. It was determined based on observation that there was a small wooden cabinet under the sink of the restroom in Room C-Right containing cleaning supplies (Scrubbing Bubbles, Bleach, Clorox wipes) that were accessible to the children. 591-1-1-.18(12) prohibits children from being in the kitchen except as part of a planned, supervised learning experience. It was determined based on observation that the kitchen door was open, leaving it accessible to the children.

591-1-1-.25(13) requires that potentially hazardous equipment, materials and supplies be stored in a locked area inaccessible to children. It was determined based on observation that there was a small wooden cabinet on the floor of the restroom in the toddler room containing cleaning supplies that were accessible to the children.

591-1-1-.18(5) requires the refrigeration of all perishable and potentially hazardous foods at 40 degrees Fahrenheit or below and served promptly after cooking. Freezer temperature shall be maintained at zero (0) degrees Fahrenheit or below. It was determined based on observation that the refrigerator and freezer in the kitchen were not cooling to the required temperatures (freezer 40 degrees Fahrenheit, fridge 55 degrees Fahrenheit,). There was an additional refrigerator in a store room that was cooling to the required temperatures. All contents were moved to this additional refrigerator.

591-1-1-.03(2) requires the Center to keep current lesson plans on site that reflect appropriate instruction practices and activities to support children's development The Center shall have sufficient and varied play and learning equipment and materials to support the above program of activities in all developmental areas. It was determined based on observation that the 1R classroom (infants) did not have a lesson plan.
