Based on observation, the licensee did not comply with the section cited above when approximately four piles of dog droppings were in the outdoor activity space, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The outdoor activity space was cleaned up immediately. Per Licensee, the outdoor activity space will be inspected prior to the children going and playing outside.
Based on observation and record review, the licensee did not comply with the section cited above when the fire extinguisher was serviced in November 2022, which does not meet the State Fire Marshall standards and poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee, the fire extinguisher will be serviced and the Licensee will make a plan to ensure the fire extinguisher is serviced each year to meet the State Fire Marshall standards
Based on interview and record review, the licensee did not comply with the section cited above in one of two staff file. S1 has not been immunized against influenza, pertussis, and measles which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee, staff will obtain the required immuizations. A copy of the proof of the immunizations will be sent to the Department.
