Based on records reviewed, the licensee did not comply with the section cited above as licensee did not ensure documentation of required immunizations were maintained for staff members (S1) which poses a potential health risk to children in care. S1 is missing documentation of proof of immunity for MMR.

Resolution: LPA discussed the requirement to maintain documentation of the required immunizations for all employees and volunteers. Licensee agrees to provide proof of documentation of proof of immunity for MMR for her assistant (S1) to LPA by 09/13/2024.

Based on record review, the licensee did not comply with the section cited above. LPA observed the infant sleep log documented the start and end time of the infant sleep time and not the 15 minute check time. Which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to check the infant while sleeping every 15 minutes, log the 15 minute check and submit a copy to the department by 08/29/2023.

Based on observation, the licensee did not comply with the section cited above. LPA observed advertisements which included magnets on the Licensee's vehicle and lawn signs that did not reveal the facility license number. Which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee added the facility license number to their vehicle magnets and lawn sign advertisements during visit.

Based on observation, the licensee did not comply with the section cited above. LPA observed a swing that was located in the backyard however, not anchored to the ground. Which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee removed the swing from the backyard during the inspection.
