Resolution: The licensee agrees to provide proof of making the cleaning compounds located in the cabinet located under the kitchen sink inaccessible to children. The proof of correction shall be submitted to CCLD on or before 6/19/2025.

Based on an interview, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee agrees to complete the renewal of her mandated reporter training and provide a copy of her certificate of completion to CCLD on or before 6/19/2025.

Based on record reviews, the licensee did not comply with the section cited above in 1 out of 6 children which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee agrees to provide CCLD current copies of immunization records for Child #1. The plan of correction shall be submitted on or by 6/19/2025.

Resolution: The licensee agrees to provide a completed notice Family Child Care Home Notification of Parent's Rights, LIC 995A for Infant #6. The plan of correction shall be submitted to CCLD on or before 6/19/2025.

Based on a record review of the licensee's roster of children in care, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee agrees to update and maintain current at all times her roster of children in care. A copy of the current roster of children in care shall be submitted to CCLD on or before 6/19/2025.

Resolution: The licensee agrees to have the parent or guardian of Infant #6 completed the Consent for Medical Attention. A completed copy of the consent shall be submitted to CCLD on or before 6/19/2025

Resolution: The licensee shall have the parent for Infant #6 complete the affidavit regarding liability insurance. The licensee agrees to provide a copy of the completed affidavit to CCLD on or before 6/19/2025.

Resolution: The licensee agrees to maintain the 15 minute sleep documentation for Infant #6 as required. The licensee shall provide copies of two weeks of sleep documentation to CCLD on or before 6/19/2025.
