Reporting requirement, during the operation of the child care center.. a report shall be made to the Department by telephone or fax within the Department's next working day.. In addition, a written report containing the information specified... Any injury to any child that requires medical treatment. This requirement was not met as evidence by: Based on LPA Corral interview with Director Taryn and record review, the facility failed to inform the Licensing office that C1 and C2 obtained medical attention. This poses a potential health risk to the children in care.
