Based on observation and licensee statment Agnes Rasolian (licensee mother) has been visiting from out of state for a week without a criminal record clearance, the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will have mother fingerprinted and live scanned tomorrow. Will email a copy of documents to LPA on 96/08/2022.

Based on observation) the carbon monoxide detector isnt functioning, the licensee did not comply with the section cited above which pose a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will add batteires to carbon monoxide detector. Licensee will contact LPA via facetime to observe detector is functioning
