LPA observed that two staff members were missing complete immunization records. One staff member was missing their MMR vaccination and the other staff member was missing their MMR and Tdap immunizations. This poses a potential risk to the health and safety of children in care.

This requirement was not as evidenced by: Lead test results received by Licensing on 3/13/2023, tested on 3/4/2023, showed an outlet in the facility had lead levels exceeding 5 parts per billion (ppb). This poses a potential risk to the health and safety of children in care.
(2) Each person specified in (g) above shall have a health-screening report signed by the person performing the screening. This report shall indicate the following:
