Resolution: The licensee stated that she will renew her CA mandated reporter training certificate and submit proof to the LPA by 03/21/25.
Based on a file review on 03/07/25, the licensee did not comply with the section cited above in 102418(g)(1). The LPA observed that child C1 and C2 did not have updated PM 286 immunization logs in their files. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee stated that she will obtain an updated immunization log from the authorized representatives of child C1 and C2 and transcribe the immunizations onto the child's PM 286 and submit proof to the LPA by 03/21/25.
Based on a file review on 03/07/25, the licensee did not comply with the section cited above in 102425(j)(D)(a-c). The LPA observed that child C1 did not have a current 15 minute nap check log in their file. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee stated that she will start documenting C1's 15 minute nap checks effective immediately and submit proof of two weeks worth of nap checks to the LPA by 03/21/25.
