Based on record review, the licensee did not comply with the section cited above in that 1 assistant did not have a criminal record clearance which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee will ensure that the assistant completes a criminal background check and will not return to the facility until they recieve clearance. Licensee will send proof to the department upon completion.
Based onrecord review, the licensee did not comply with the section cited above in that 2 of 2 licensees had expired mandated reporter certificates which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee's will complete mandated reporter training and will send certificates to the department by the POC due date.
