Based on observation, the licensee did not comply with the section cited above in that two palm tress were found with thorns in the backyard which were accessible to children which posed a potential health, safety or personal rights risk to persons in care.
Resolution: Staff will cut thorns off of the palm trees and will do frequent check to ensure that thorns are cut if they grow back. Licensee will also add fencing around trees to make them inaccesible to children.
Based on record review, the licensee did not comply with the section cited above licensee and assistant were missing or had expired mandated reporter training, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: LIcensee stated training will be updated and proof provided to LPA.
