Based on interview and record review, the licensee did not comply with the section cited above, the last fire drill was documented on 8/11/22. Which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Within two weeks, licensee will email LPA Chancellor a photo copy of an up-to-date fire drill. Moving forward Licnesee agrees to complete and document drill at least every six months.
Based on observation and record review, the licensee did not comply with the section cited above in 2 staff members missing thier certificate. This which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Email LPA Chancellor copys of mandated reporter certificated for both general training and child care provider. Maintain trainings every two years.
