Based on observation, the licensee did not comply with the section cited above in poisons were accessible to children in care inside and outside in the unlocked shed which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee locked the shed with a padlock. POC cleared on site.

Based on observation, the licensee did not comply with the section cited above in infant observed sleeping in a swing with a blanket covering the child which violates safe sleep regulations which poses an immediate health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in C4 did not have current 15 min sleep checks on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will obtain proof of child's immunizations and send documentation to LPA Sims via text (916)365-5713.

Based on observation, the licensee did not comply with the section cited above in having posions (weed killer, gasoline) accessible to children in the backyard and not locked. which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will move all posions into back yard shed and lock the shed with a padlock. Licensee will send proof of poisons locked in shed via text to LPA Sims at (916)365-5713.

Based on record review, the licensee did not comply with the section cited above in 1 count of not maintaining 15 minute infant safe sleep log which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will create a safe sleep log and document infants 15 minute checks for 11/21/23 and 11/22/23. Licensee will send documentation to LPA Sims via text to (916)365 - 5713.

Based on observation, the licensee did not comply with the section cited above in one count of not having a charged fire extinguisher which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will obtain a fire extinguisher that is rated at least a 2-A-10-BC and charged. Licensee will send proof of fire extinguisher to LPA Sims via text to (916)365 - 5713

Based on record review and interview, the licensee did not comply with the section cited above for one adult, A1, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee immediately called and made an appointment for A1 to have fingerprints completed on 6/21/22.
