PARAPITIYA FAMILY CHILD CARE

Based on record review, the licensee did not comply with the section cited above for C2. The sleep log for C2 indicates a last entry as June 3, 2025 however C2 has been in care following that day and there are no additional entries. This poses a potential health, safety or personal rights risk to children in care.

Resolution: Licensee will submit a written declaration of understanding of the regulation cited and the importance of maintaining accurate sleep logs for all infants in care. The declaration will be submitted to the Department by 5:00 PM on June 30, 2025. Once received, LPA will clear the violation and send a cleared deficiency letter to Licensee.

Based on LPA Wilburn's observation, the licensee did not comply with the section cited above in the Fire Extinguisher was observed to not be Full and in the "Recharge" Area, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee advised she will contact her local fire department, to inquire about getting the fire extingusher serviced, and if they do not procide the service, she will purchase a new one. Either way, Licensee will submit proof of correction to LPA by plan of correction date of 06/24/2022.
