DIAZ FAMILY CHILD CARE

Based on observation, the licensee did not comply with the section cited above in LPAs observed a sleeping child (C1) on the couch in living room, front door unlocked, door alarm not on and out of view of adult supervision which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensees agree to create a plan of correction to include entry into the FCCH, supervision of children in the front living area at all times and provide a copy to LPA Hamilton via email on or before the POC due date.

Based on observations, the licensee did not comply with the section cited above in the backyard playarea had bushes with thorns, chipped wood on the play structure, chipping paint on play structure, fire pit without a cover which poses/posed a potential health, safety or personal rights risk to persons in care

Resolution: Licensee will have play structures sanded and painted and will remove the bushes with thorns. Licensee will provide pictures to LPA via email on or before the POC due date.

Based on observation, the licensee did not comply with the section cited above in there were scissors in first aid kit accessible to children in the bathroom, uncovered electrical outlets in daycare area and living room and a knife in an unlocked kitchen drawer which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee placed first-aid kit in locked bathroom vanity and made knife inaccessible during the inspection. Licensee also covered the electrical outlets

Based on observation, the licensee did not comply with the section cited above in sheets in two play yards were not tight fitting which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to purchase tight fitting sheets for the play yards and provide proof to LPA via email on or before the POC due date.

Based on observation, the licensee did not comply with the section cited above in play yard was placed under a guitar hanging on the wall which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed the play yard from under the guitar hanging on the wall during the inspection and licensees agree to not place the play yards underneath in the future.

Based on record review, the licensee did not comply with the section cited above in S2 and S3 Mandated Reporter Certificates are expired which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have S2 and S3 complete the mandated reporter training and provide proof of completion to LPA Hamilton on or before the POC due date via email.
