Based on record review, the licensee did not comply with the section cited above in 2 of 5 staff were missing mandated reporter certificates which poses/posed a potential health, safety or personal rights risk to persons in care.
Child Care Center Directors Qualifications and Duties- (b) All child care centers shall have a director. Based on interview and record review, the center did not comply with the citation noted above. The center has been without a qualified director since October 9, 2025 which poses a potential health, safety, and personal rights risk to children in care.
Reporting Requirements (b) The name of the child care center director, and any fully qualified teacher(s) designated to act in the child care center director's absence, shall be reported to the Department within 10 days of a change of child care center director or designee(s).
Based on observation, the licensee did not comply with the section cited above in that LPA found one persciption medication stored without the original label which poses/posed a potential health, safety or personal rights risk to persons in care.
