Based on record review, the licensee did not comply with the section cited above in 2 of 5 staff were missing mandated reporter certificates which poses/posed a potential health, safety or personal rights risk to persons in care.

Child Care Center Directors Qualifications and Duties- (b) All child care centers shall have a director. Based on interview and record review, the center did not comply with the citation noted above. The center has been without a qualified director since October 9, 2025 which poses a potential health, safety, and personal rights risk to children in care.

Reporting Requirements (b) The name of the child care center director, and any fully qualified teacher(s) designated to act in the child care center director's absence, shall be reported to the Department within 10 days of a change of child care center director or designee(s).

Based on observation, the licensee did not comply with the section cited above in that LPA found one persciption medication stored without the original label which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on LPA Wilburn's record review, the licensee did not comply with the section cited above in 3 out of 4 Staff, Staff #1(S1), Staff #2 (S2) and Staff #4 (S4) do not have proof of a health screening and S2 does not have proof of a health screening and TB test with results available for review at time of visit, which poses/posed a potential health, safety or personal rights risk to persons in care.

(1) No child(ren) shall be left without the supervision of a teacher at any time... Supervision shall include visual observation. This requirement is not met as evidence by: Based on interviews conducted and LPAs observation, staff failed to properly supervise children on 05/05/22, where a parent observed 4 boys in the restroom unsupervised, which poses a potential health, safety or personal rights risk to children in care.

Based on record review, LPA Wilburn observed the licensee did not comply with the section cited above in 4 out of 8 Teacher Aides, S5, S6, S7 and S8 did not have proof of transcripts to verify units and/or credits in Early Childhood Education or Child Developments at time of visit, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review, LPA Wilburn observed the licensee did not comply with the section cited above in 4 out of 8 staff had incomplete files and were missing required documents, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on LPA Wilburn's observation, the licensee did not comply with the section cited above in the facility water system has not had lead exceedance testing, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on LPA Wilburn's record review, the licensee did not comply with the section cited above in 1 out of 4 Staff, Staff #2 (S2) did not have proof of required immunization's available for review at time of visit, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above in that LPA found an exposed rusted screw and nail on a piece of the wall accessible to children which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above in that lysol spray and wipes were out in the classroom accessible to children which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review, LPA Wilburn observed the licensee did not comply with the section cited above in 6 out of 8 Staff, S2, S3, S4, S5, S6 and S7 did not have proof of immunization's at time of visit, which poses/posed a potential health, safety or personal rights risk to persons in care.

The Director has not been actively present and the substitute has been gone for two months. The current substitute is not fully qualified, which poses a potential health and safety hazard.
