Based on record review, the licensee did not comply with the section cited above in 1 out of 4 staff without current mandated reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.
LPA observed a 3 inch x 3 inch hole in the screen located in the Kindergarten room, leading out to the driveway and street. This poses a potential health and safety risk to the children in care.
(1) The center shall give each authorized representative a copy of the Personal Rights form (LIC 613A [9/96]).
Licensee did not provide sheets for the cots for children napping. LPA observed 3 napping children on cots with no sheets covering the cots, and 8 other cots with no sheets covering cots as well. This poses a potential health and safety risk to the children in care.
