Based on interview and record review, the licensee did not comply with the section cited above in 2 out of 2 persons (licensee and assistant) with expired Mandated Reporter Training which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states she will have her and her assistant take the Mandated Reporter training for Child Care Providers and email certificates of completion to LPA.
Based on interview and record review, the licensee did not comply with the section cited above in 2 out of 2 persons, licensee and assistant without Pediatric CPR/1st aid which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states she and her assistant are registered for a Pediatric 1st Aid/CPR class with the American Heart Association to be held on 10/5/24.
Based on interview and record review, the licensee did not comply with the section cited above in 2 out of 2 persons in the facility (licensee and assistant) missing proof of immunizations, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will obtain proof of immunizations for herself (pertussis) and her assistant (measles, pertussis, flu, and negative tuberculosis test) and email proof of immunizations and test to LPA.
