Based on record review, the licensee did not comply with the section cited above in 1 out of 2 staff did not have a mandated reporter training cerificate, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete mandated reporter training and mail it to the department.

Based on record review the licensee did not comply with the section cited above as Licensee doe not have proof of Preventative Health and Safety training, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete training and email certificate to LPA.

Based on interview and record review, the licensee did not comply with the section cited above in 1 of 1 infants without a complete sleep log documenting 15 minute sleep checks, which poses a poential health, safety or personal righs risk to persons in care.

Resolution: Licensee states she will begin using a sleep log for the infant in care doucmenting 15 minute sleep checks, beginning 3/26/25 and going forward with any infants in care under the age of 2.

Based on observation and interview, the licensee did not comply with the section cited above in 1 out of 1 fire extinguishers not serviced or with proof of purchase within the last year, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will purchase a new fire extingusiher and email LPA a copy of the receipt.

Based on interview and record review, the licensee did not comply with the section cited above in 2 out of 2 staff with no current Mandated Reporter Training, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and Staff #2 will complete Mandated Reporter Training and email certificates to LPA.

Based on interview and record review, the licensee did not comply with the section cited above in 1 out of 2 staff with no proof of measles, pertussis, or influenza immunizations and no proof of negative TB Test, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will have Staff #2 obtain proof of immunizations or immunity for measles, pertussis, and influenza (declination OK for influenza) and email to LPA.

Based on interview and record review, the licensee did not comply with the section cited above in 1 out of 1 staff without proof of Preventative Heath and Safety Training by an EMSA approved training company, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will enroll and take Preventative Health and Safety Training, including Nutrition and Lead, from an EMSA approved training company and email the certificate to LPA.

Based on interview and record review, the licensee did not comply with the section cited above in 2 out of 2 infants with no sleep log documenting 15 minute sleep checks, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states she will begin using the sleep logs for both infants in care documenting 15 minute sleep checks, beginning the next time they are sleeping and going forward with any infants in care under the age of 2. Licensee will email sleep logs for both infants in care to LPA for the dates of 5/8/23 - 5/12/23.
