Based on observation, the licensee did not comply with the section cited above in that LPA observed cleaning solutions and poisons in an unlocked cabinet in the bathroom that children use which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee, they will add a safety lock to the cabinet and submit pictures of lock- a written plan will be submitted on how they will maintain compliance. Poisons were removed during inspection and placed in off limits area.

Based on observation, interview and record review, the licensee did not comply with the section cited above in that they are not documenting the sleep of child # 3 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee, they will begin documenting child's sleep today, and will have documentation available for review by POC due date of 3/31/2025

Based on record review, the licensee did not comply with the section cited above in that child # 3 is missing complete file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee, they will request documentation from parents and have file available for department review by poc due date

Based on observation, the licensee did not comply with the section cited above in 1 out of 1 carbon monoxide was inoperable which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will purchased and install carbon monoxide by 04/12/2023.

Based on record review, the licensee did not comply with the section cited above in one out of one infant in care with no sleep lpg which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will begin using sleep log to document child's sleep every 15 minutes as of tomorrow until she is 2 years of age.

Based on interview and record review, the licensee did not comply with the section cited above in 1 out of 1 staff files reviewed with no proof of mandated reporter which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will compete mandated reporter training on line and will email certificate to LPA.
